CDSA Plastic DVD Packaging Certification Program -
Fee Schedule

Step one: program application
Receipt of your application and fee is the first step in the certification process.

Program Fee (per plant)*:
CDSA Member Company - $5,000 US
CDSA Non-Member Company Rates - $7,500 US

Fees are payable with the Plant Application for Certification for admission into the Plastic DVD Packaging Certification Program.

*The Program Fee, and annual fee paid with subsequent certification, will underwrite an advertising campaign listing certified manufacturers of compliant Plastic DVD Packaging.

After your application is reviewed and accepted, CDSA will deliver a formal copy of the Plastic DVD Packaging Standard 1.0 for your plant’s internal review.

Step Two: Initial Certification Phase
The Initial Certification Phase is valid for one year.

The Initial Certification Phase requires plant applicants to develop and establish internal protocol for compliance as defined in the Plastic DVD Packaging Standard 1.0.  Said protocol will establish that each plant’s personnel shall conduct an internal audit to ensure continued compliance to this Standard.

Additionally, each plant must contact and establish a business relationship with a laboratory that is certified by either the American Association for Laboratory Accreditation (A2LA) or the International Laboratory Accreditation Cooperation (ILAC).  Proof of laboratory certification must be provided to CDSA as part of the Initial Certification Phase.

The applying plant will then submit samples to the laboratory for testing and analysis in accordance with the Standard.  A minimum of three (3) plastic DVD packaging cases are to be submitted to the laboratory in accordance with the Standard.  Results of testing will be sent directly to CDSA from the testing laboratory for review and acceptance.

By the end of the Initial Certification Phase, after samples have been submitted to the laboratory and results sent directly to CDSA, the applying plant will provide to CDSA a written summary of their internal audit in accordance with the Standard.

Upon receipt of this written summary and review of initial test results, with confirmation that the samples have met the required minimums in accordance with the Standard, CDSA will issue certification to the plant that will be valid for one year.

Step Three: Program Continuation Annual Fee
After the initial Certification Phase of the program (six months) and subsequent certification, the plant continues its certification in the program with an annual submission of samples to the certified laboratory with analysis results sent directly to CDSA.    Each plant’s personnel shall conduct an annual internal audit to ensure continued compliance to this Standard. A report of the findings of each internal audit and the results of any corrective actions shall be made in writing to CDSA.

The Annual continuation fees are:
CDSA Member Company - $2,500 US
CDSA Non-Member Company Rates - $5,000 US

Note: In the case of major or systemic non-conformities, the plant must undergo an external surveillance audit performed by a CDSA auditor.  A minimum charge of $2,500 per audit applies.
