Agreement

  • Certification under the CDSA Plastic DVD Packaging Certification Program is valid only for individual plants, not entire companies.  Each plant within a company must apply separately for certification.
  • Plants must complete and submit a Plant Application for Certification form along with the required fee that covers the various costs of the program, including certification review and marketing for the program.
  • A mutually agreed upon implementation schedule is to be developed within 60 days of the application being accepted. Generally, the full certification process is expected to take between 4 and 6 months.
  • Each plant must contact and establish a business relationship with a laboratory that is certified by either the American Association for Laboratory Accreditation (A2LA) or the International Laboratory Accreditation Cooperation (ILAC).  Furthermore, said laboratory will be instructed to send copies of testing results directly to CDSA for compliance review.  Documentation from the laboratory will specifically state the quantitative elemental analysis data obtained from the laboratory examination of finished plastic DVD packaging components.  Copies of this documentation will also be sent to the plastic DVD manufacturer to be held for their records.
  • A minimum of three (3) plastic DVD packaging cases are to be provided as samples for laboratory testing.  These samples will be pulled, at random and during a legitimate production run, from at least two independent tools utilized in the production process by the plastic DVD packaging manufacturer.  Should the plastic DVD packaging manufacturer have less than two tools in the production operation the three (3) samples will be pulled from a single tool.  After initial certification, testing will be done on an annual basis.
  • The manufacturer shall establish, maintain and implement annual internal auditing procedures related to both the pulling of samples and procedures used to submit for laboratory testing to ensure their products comply with the CDSA Plastic DVD Packaging Standard 1.0.
  • From the time of acceptance of the Plant Application for Certification, the plant has one year to complete the certification process.  If one year has lapsed and the plant is not certified, the plant must re-apply for entry into the program and pay the appropriate fee. Certain conditions may be applied for readmission.
